Please read  and follow these steps carefully to be fully registered.  

IMPORTANT:  Once all steps are complete you are cleared to participate. This process is required prior to the first day of your sports season for all athletes in grades 7-12 for the 2016-17 school year.

 

Certification as to the adequacy of a student’s health for athletic participation shall be restricted to a duly licensed doctor of medicine, doctor of osteopathy, doctor of chiropractic, licensed physician’s assistant or licensed nurse practitioner.  Stamping the name of a medical clinic or a medical association as a substitute for the Doctor’s signature is unacceptable. All exams must be signed by authorized medical personnel as listed above.  Physicals taken in the spring for the ensuing school term shall be taken after April 1.  For example, a physical taken on April 1, 2016 is valid for the 2016-17 school year.  A physical taken on March 1, 2016 expires on July 1, 2016 and is not valid for the 2016-17 school year.

 

Participants in all middle school and senior high sports programs must complete Steps #1- #4 unless noted below. 

Once you have you have completed all required steps for your activity or activities, you can turn all completed material into the Harrisburg HS office or you can scan it and submit completed material HERE. INCOMPLETE MATERIAL WILL NOT BE ACCEPTED, MAKE SURE THE ENTIRE PACKET IS COMPLETE.

Step 1Athletes must fill out the pre-participation physcial evaluation history form prior to seeing the health care provider, the physical portion will be filled out by the health care provider. Download the 2016-17 forms HERE.

Step 2Athletes must have a current sports qualifying physical exam on file in the Activities Office.  

Step 3:  Fill out the Harrisburg Student Activities Participation Form HERE:

Step 4:  Submit completed physical form HERE (or return it to the office) prior to the first practice.  You will not be allowed to practice until all paperwork is turned in.
